Friday 8:30-4:30 for patient questions by phone or email.

After hours please contact your PCP or in case of emergency dial 911 or go to Urgent Care or ER.


New and current patients may go straight to the PATIENT PORTAL to conveniently and securely request an appointment.  If you’d prefer, you may also call 802-857-5671 to make an appointment.  You will be asked for your insurance card information when you call.

Note: We require 24 hours notice to cancel an appointment.

Please bring to your appointment:

  • Current insurance card
  • List of current medications and medical conditions
  • Primary-care physician address and phone number and contact information for other physicians involved with your care
  • Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs

Patients who are new or who have not visited our offices in more than a year should complete the patient registration form on the PATIENT PORTAL. If you do not pre-register on the patient portal, you will need to arrive 20 minutes before your appointment time to complete paperwork.  This will expedite your check-in.
